Caramelize the Onions
Heat butter and olive oil in a large skillet over medium-low heat. Add onions and a pinch of salt. Cook 25–35 minutes, stirring occasionally, until deeply golden and caramelized.
Add Garlic
Stir in garlic and cook 30 seconds until fragrant.
Deglaze
Pour in broth and scrape up browned bits. Simmer until slightly reduced.
Cook Pasta
Meanwhile, cook pasta in salted water until al dente. Reserve ½ cup pasta water, then drain.
Make the Sauce
Lower heat and stir in cream. Warm gently until silky.
Combine
Add pasta to the skillet. Toss to coat, adding reserved pasta water if needed.
Finish
Remove from heat. Stir in Parmesan, season with pepper, and garnish if desired.
